SophiMed Membership & Packages

SophiMed Membership Includes:

• 2 hour initial welcome visit in person with basic labs included
• 1 hour visit thereafter with basic labs included
• Choice of Telemedicine visits (usually 30 minutes – better for quick update, illness or just bad weather!  We know that happens)
• Unlimited visits per month (in person, telemedicine or combination)
• 2 full exams per year, 2 hours (includes full labs including cholesterol, EKG, Bioscan)
• Sophisticated EMR with Interactive Patient Portal
• 24 hour messaging (although after 6pm, answered next day)
• NO messy insurance (although can use for imaging, specialty labs, etc)
• Expert Coordination of Care with other Providers locally and nationwide
• Online patient scheduling
• Starting in May will have 3 month minimum
• Auto Debit available for ease of payment
• Discount on One-and-Done for family visiting from out of town
• Discount on all Events (more to come here) – our first EVENT 2/18.

Not Included in Membership Fee:

• All Xrays, CT scans, MRI, Specialist Appointments, and Ultrasounds (although if you don’t have insurance, will negotiate a cash pay price for you)
• Complex lab panels (will go through insurance)
• Some Functional Medicine labs (but you will know cost ahead of time and often payment plans through Rupahealth are available)
• All treatments outside of our office (other providers, hospitlization, ED visits, things we don’t have a direct involvement in).

One-And-Done – If You’re Not Ready for Membership

• One-and-Done available – $175/visit
• No telehealth option, just in office
• If you decide to become a member, the $175 goes toward first month.

Heart Health Package (4 months) – 3 visits with Provider

1st Appointment

• Get a Comprehensive Medical History
• Assess Cardiovascular Risk
  • Complete Metabolic Evaluation
  • Waist to Hip Ration
• EKG – Look at Nutrition & Lifestyle
• Order Labs
  • Basic labs CBC, CMP, Ferritin, Thyroid, Cholesterol Panel.)
  • Cardiac Specific labs – Stool testing/Food Allergies

2nd Appointment (2weeks later)

• Review your labs
• Assess & Devise Treatment Plan

** If you have PCP and/or Cardiologist, Vascular Surgeon; we won’t start any treatment plans w/o their approval.

3rd Appointment (6 weeks out after start Rx)

• Review symptoms.
• Redraw labs, see where you are
• Review diet
• Review how things are going

$475 (incl labs if done at Quest by Health Dept 119)

Gut Package

Included w/ Package:

3 visits (all visits and labs are included in package price)

• Lab draw
• Basic labs
• Colon Health Labs
  • Stool Studies
  • Food Allergies
  • CRP (mark of inflammation)
  • and others depending on intake history.
• Assess lifestyle risks
• Complete Gut Health Evaluation
  • Assess Symptoms of Bloating, Fatigue, Brain Fog, etc.

1st Appointment

• Get a Comprehensive History and Gut Health Evaluation
• Assess Symptoms Profile
• Assess Family History
• Assess lifestyle
• Get lab orders sent in – Labs have to be done at Quest on M-119 so you don’t have to pay.
• Included if you go to Quest (It’s by the Health Department on 119).
• Assess your medications and supplements you are already taking.

2nd Appointment – 2 weeks after labs drawn

• Review your labs
• Assess options:
  • If you have a PCP, will share ALL of this information with them if you’d like.
  • If you don’t have a PCP, but don’t want to become a **I would then like you to follow up in 3 months to review your meds (if started any), redo labs, redo bioscan, see how you’re doing. This would be $175.member, we will go through everything, discuss options, lifestyle changes, nutrition, activity, supplements, possible meds.
  • Place any needed referrals if symptoms/labs/etc leaning toward needed Gastroenterology or other.
  • Set up an Screening if needed (Colonoscopy, Cologuard, etc)

3rd Appointment – about 4-6 weeks later

• Assess symptoms
• Redraw labs if needed
• Close the loop on how you are doing.

I would then like you to follow up in 6 months to review your meds (if started any), redo labs, redo bioscan, see how you’re doing. This would be $175. But it is not mandatory.

• Go through labs and hormone levels
• Discuss options based on risks and goals
• Assess where you are in Perimenopause, Menopause or Andropause.
• Discuss treatment options

3rd Appointment: (4-6weeks later)

• Assess symptoms, how you’re feeling
• Redraw labs (Telehealth in 1 week to go through unless feel need in person appt)
  discuss long term strategies
• handoff to you and your PCP if you have one. You can always continue to follow up with us, but at this point there is no obligation to do so.
